2008 Star Mazda Championship presented by Goodyear
DRIVER REGISTRATION AND ENTRY INSTRUCTIONS
Attached you will find all the necessary forms to complete before you will be eligible to test or race with the Star Mazda Series for 2008.

Series Registration Form - Please complete this form if you are intending to enter the Star Mazda Championship Presented by Goodyear and accumulate points towards the Championship.
Race Entry Form - Each driver who wishes to enter a race will need to complete a Race Entry Form for each event he or she is entering. One race per entry form with the exception of the Full Season Entry. Teams are allowed to complete these forms for the drivers.
Mazda Release Form - Each driver will need to read and sign this release form before you will be eligible to test or race with the Star Mazda Series.
Publicity Authorization - Each driver will need to complete this form to be eligible to test or race with the Star Mazda Series.
Event Medical - Each driver will need to complete this form before he or she will be eligible to test or race with the Star Mazda Series. This is a Star Mazda Series form different from the IMSA medical.
FCC License - All teams, crewmembers or independent drivers must apply for an FCC License if using FM business band radios.
IRS Form W-9 - All drivers who reside in the United States must complete IRS form W-9. These must be submitted with Series Registration Form to be eligible for prizes.
IRS form W-8 - Drivers who do not reside in the United States must complete IRS form W-8. These must be submitted with Series Registration Form to be eligible for prizes.
VIP Catering Form - Each driver will receive two wrist bands for catering with each race entry for each day the cars are on track. Please complete this form for any additional bands you may require for each day of each event. We appreciate your request at least 10 days prior to event.
